If you feel that you may be in the need of physical therapy and you do not have a referral, you may make an appointment for an assessment only. After the assessment, your physician will be contacted for an appropriate referral, if deemed necessary.
After every evaluation, a plan of care is dictated and sent to your referring health care provided that will include our evaluation findings, goals to be met, planned treatment ideas, and frequency of visits. This plan of care is a regulatory component of the rehabilitation services and also serves as documented communication between health professionals.
